EQUIFAX (ACA FORM 1095-C)
As part of the Affordable Care Act, Evonik provides pre-65 retirees a Form 1095-C annually. This form contains detailed information about your and your dependents’ Evonik medical coverage While the information on this form may assist in preparing your tax return, it is not required.
The 1095-C form is available to you electronically via Equifax at https://www.mytaxform.com/.
When you access the Equifax site, you will need to put in Evonik as the company name or use the code 14231 and then follow the prompts to register. You will also need to provide your consent to receive the form electronically. You can provide this consent by scrolling down and clicking on “Delivery Selections” and then consenting in the “Online Delivery Consent Section”. Once you provide consent to receive your 1095-C electronically, all future 1095-C forms will only be available electronically. You will no longer receive a mailed copy to your home address.
